All LCPS student-athletes must have a VHSL (Virginia High School League) Pre-Participation Examination prior to the start of their sports season including any off-season workouts to be eligible to try out for sports during the 2026-2027 school year. This sports physical expires 14 months from the date of the practitioner's signature. This physical can be completed by any MD, DO, PA or NP currently licensed in the US.

All proceeds from the sports physical will go directly to the Athletic Training program for that student-athlete's current or future high school. This donation is typically less than the cost of having a physical completed at a medical center or a walkin-in outpatient clinic.

All the healthcare professionals providing these sports physicals are volunteering their time and expertise. LCPS student-athletes may attend any of the locations and their paperwork and donation will be transferred to their high school.

Please print and complete the VHSL Athletic Participation/Parental Consent/Physical Examination Form prior to your arrival at the sports physical location. Copies may also be picked up at any of the LCPS high school's athletic departments.
